The Community Health Coordinator
(CHR) program provides a variety of services to the
community.
Coordinates patient travel to medical appointments in
Elko, Nevada or Salt Lake City, Utah.
Transportation is primarily offered to
individuals who lack reliable transportation.
The CHR also conducts home visits to
individuals who require monitoring because of a medical
issue, provides community educational meeting and training.
Dispenses Over-the-Counter (OTC)
medication and coordinates patients scheduling for the
weekly doctor visits.
